I agree that a standard needs to exist. I would go further and say that
penalties also need to exist for those who would violate the standards.
However, I just don't know what a definition of "out of context" would
include. I like your "definition", but would ask at what point we look at to
determine the author's conclusion. Also, what if the author changes his/her
mind in a later book or article, is the first quote now out of context? 

I think, however, that you and Glen are on the right track. And while I may
not join in the discussion as often, I do agree with the need.
